Dear Shaken Baby Syndrome Conference Attendees,
 
Thanks so much for joining us on Monday for the Second New York City Conference on Shaken Baby Syndrome.  We had a fantastic day, and we hope you did too.  If you need contact information for any of the speakers, get in touch with us and we will help you reach them.  Thanks for devoting your time (a long day, to be sure!) to learning the tools to achieve justice for children. 
 
Attached below is a CLE form.  If you did not get this in your tote bag and you want CLE credit, just print it out and fax it to Carla Martinez at 718-286-5820.  If you have people in your Bureau or Agency who attended, who may not be on our original List, PLEASE FORWARD THIS TO THEM
 
To obtain CME credit, you must completely fill out an online evaluation and attestation form by November 9, 2009.  Certificates will be mailed out by November 23, 2009 and should be received within 1-2 weeks.  Please note that the evaluation should be completed now.  Anyone attesting to the full 7.25 hours of the course before it is completed may not receive CME credits until their attendance can be completely verified.
